What travels between practice and lab – and why appointments depend on it

A dental lab runs on its shuttle traffic with the practices it serves. In the afternoon, impressions, bite registrations and shade notes come out of the surgeries; in the morning, frameworks for try-in, wax set-ups and finished crowns, bridges or dentures go back. Every piece is a custom-made device for exactly one patient, and almost every one is tied to a fixed appointment: a try-in, a fitting, or the repair of a broken denture without which the patient cannot eat.

What goes wrong is rarely the route itself but the handover. The bag ends up at the wrong reception, an alginate impression dries out in a warm car, a plaster model cracks in a loose crate, and later nobody can say when the work actually reached the practice. Sending lab work through parcel services or shared rounds also costs time in depots and sorting – time that is missing between impression and fitting.

  • Cologne: daily practice round from Ehrenfeld

    A lab in Cologne-Ehrenfeld has its practices in Lindenthal, Sülz and Nippes visited every weekday afternoon. At each reception the driver collects the disinfected, sealed impressions and bite registrations and takes them to the lab. Next morning the round runs the other way with finished work. Within the city the cargo bike or a car does the job. The practice only needs a fixed tray at reception and someone to sign on the driver's device.

  • Düsseldorf: same-day denture repair

    In the morning a patient's upper denture breaks at a practice in Oberkassel. The practice cleans and disinfects it, adds the repair order and books a direct drive to the lab in Bilk. Once the repair is done, the lab books the return run; both see the earliest available pickup window immediately when booking. At reception the practice assistant signs, and the proof of delivery with the time sits in the lab's account. At best, the patient is wearing the denture again the same day.

  • Münster: framework try-in for a long-span bridge

    For a multi-unit implant bridge, a lab in Münster sends the framework to a practice in Hiltrup for try-in. The framework travels in a rigid lab box with the model and order slip. After the try-in, framework, new bite registration and the dentist's notes go back to the lab on a return run so the veneering is ready before the fitting appointment. A car is enough. What matters is that the practice books the return run right after the try-in, not in the evening.

What has to be settled before pickup

The drive is the smallest part. Practice and lab settle these points in advance – Maxmove carries what is handed over sealed and labelled.

  • Disinfection before it leaves the practice

    The 2006 recommendation on infection prevention in dentistry by KRINKO, the hygiene commission at the Robert Koch Institute, classes impressions, bite records and dental work pieces as microbially contaminated. It recommends releasing them only after cleaning and disinfection with suitable methods and calls for specific agreements between practice and lab. Responsibility lies with practice and lab; the right disinfectant for each impression material is in the manufacturer's instructions. The driver only accepts sealed shipments and opens nothing. A note saying disinfected, with the date, on the bag saves the lab a phone call.

  • Packaging to suit the material

    Under Section 411 of the German Commercial Code (HGB), the sender packs goods so they are protected against loss and damage. For labs that means one case per bag or box, plaster models and ceramics in rigid, padded lab boxes, nothing loose in the transport crate. Alginate impressions are sensitive to moisture loss; how long they may wait before pouring is set by the material manufacturer. That decides whether the regular round is enough or an urgent run is needed. For fixed routes, a sturdy compartment box that shuttles between lab and practice pays off.

  • Dental work is a custom-made device

    Under Article 2(3) of the Medical Device Regulation (EU) 2017/745, dental restorations are custom-made devices: made to a written prescription for a single patient, with the lab as manufacturer. Article 21(2) MDR requires the device to be accompanied by the statement set out in Annex XIII. For transport this simply means the statement goes into the sealed shipment together with the work and the lab order. Issuing and completing it is the lab's job; the courier does not check paperwork.

  • Patient data and confidentiality

    Dentists are bound by professional secrecy under Section 203 of the German Criminal Code, and a lab order with name and findings contains data concerning health within the meaning of Article 4(15) GDPR. The outside of the bag should therefore show only practice, lab and an order number or patient initials. For the custom-made device statement itself, Article 21(2) MDR also allows identification by name, acronym or numerical code. Order slip and documents go inside. The driver sees nothing he does not need to, and the proof of delivery still records every handover.

Which vehicle for which lab run

The cargo bike carries single bags and small boxes within central Cologne or Düsseldorf. The car is the standard vehicle for practice rounds and urgent runs between districts and cities. You need the compact van when many transport boxes travel at once, or when equipment such as a ceramic furnace and model trimmer has to move to new lab premises.
